There is a double <html> declaration in the document. It looks as if
the <html> declarations of both the head.inc and the overboard.php file
are being echoed into the output:

There are nested <p> tags in the tables that don't play nice.
Somewhere is probably a dangling closure tag shifting the tables to the
right like a staircase. Also the table is only occupying half the page
when it should be full page.
I also suggest eventually replacing all tables with <div> and <span> and
using display: inline-block for cells and display: block for rows so
that nested <p> tags are not an issue. Or you can give them custom tag
names since new HTML standard allows you to use any tag name you wish.

The <body> tag of the overboard table is output after the closing
</html> tag of the former part of the document. So there are two HTML
documents being output as one (see lines 32-33 of document).
There is also a <hr> tag before the closing </head> tag.

After doing another install from scratch I over wrote the
config and spool files and redid them manually. This other install does
not have the formatting errors.
I suspect something in config or spool is triggering the bug. I am
going to leave the broken example as is for later inspection.
